Ontario Hospital Report Card

The report card by provides quality of care information directly to patients and the general public. This allows you to compare acute care hospitals in Ontario and helps you make informed choices about your health care as well as improving hospital performance through enhanced transparency and accountability.

You can look up a hospital or a procedure, and compare 50 measures of patient safety and quality of care, such as death rates, the number of procedures, the rate of adverse events, and usage rates. You can also compare patient care for the municipality you live in with other municipalities.

We’ve also developed a Hospital Mortality Index as a summary of mortality rates for the larger hospitals.


Seeking Advice of Your Physician
Deciding on treatment options and choosing a hospital are decisions that should be made in consultation with a physician. It is not recommended to choose a hospital based solely on statistics and descriptions such as those given in this report.

Hospitals and Patient Records
In our rating and ranking, more than 8.5 million patient records were analysed for the Hospital Report Cards. Data from all of Ontario’s acute care hospitals are included in the report.

Forty-three out of 136 acute care hospitals, representing 41% of inpatient records in the most recent year of data, agreed to have their institution identified in this first edition of the Hospital Report Card. Other hospitals are anonymously shown in the report by number.

These hospitals should be commended for their efforts to empower patients with information regarding the health care they receive and for their ongoing commitment to quality improvement through accountability and transparency.
